We work with you to find out how much the injured person is able to do and compare that with their abilities before the injury.
A customised plan is drawn up with the therapist, the injured person, and their family or relevant other people. This plan focuses on the individual’s levels of independence and safety as they resume participation in their life roles.
Case Management for Adults & Children with Severe Injuries

This process commences with a comprehensive assessment of the current needs of the injured person.
The plan forward is discussed with the Case Manager to facilitate the safe and timely return to participation in the person’s life roles.
This process includes sourcing and making a referral to other specialised rehabilitation therapists that may be required to assist with the person’s recovery and resettlement. This can be a challenging exercise, however knowledge and experience with local and available therapists greatly assists this process.
